Accident investigation: Crew and pilot broke rules
Though the 'Leda Maersk' was equipped with modern navigation instruments, on June 10, 2018, the ship got off its route in the last turn before the port of Otago and ran aground. The Transport Accident Investigation Commission (TAIC) stated in its accident report that neither the pilot nor the crew on the bridge had kept an eye on the E-navigation, which clearly indicated that the ship had left the planned sailing route in the middle of the channel. The TAIC concluded that Maersk's internal rules and procedures for navigation in pilotage were broken, and that the pilot violated the corresponding New Zealand rules.
