Report gives insight into fatal accident
Following the fatal accident on board of the 'Morning Lisa' in Bremerhaven on Jan 11, 2024, the Federal Bureau of Maritime Accident Investigation (BSU) has now published a report according to which the crew of the ship, which was moored at Auto Quay 2, had prepared materials for loading vehicles and planned to take a break on board. Meanwhile, the boatswain intended to drive an on-board forklift up to the next deck. Because metal on board was possibly covered in frost, the forklift slipped around 10 a.m. and tipped over for an unknown reason. The boatswain suffered fatal head injuries when he was trapped under the roll bars of the driver's cabin. A crew member on watch who observed the incident from a distance immediately alerted the remaining crew. They promptly initiated first aid measures and called an ambulance and the police. Emergency services recorded the accident and conducted a brake test on the forklift. However, the test failed. The ship's command was also able to present maintenance logs confirming regular inspections of the forklift. Two investigators from the Federal Bureau of Maritime Casualty Investigation (BSU) arrived that same afternoon and began their on-site investigation and interviewing the crew. The BSU has now completed a complex investigation into the accident and is currently working on a detailed investigation report.
