Tug allided with boom

The 'Ocean A. Gauthier' allided with the unloading boom of the berthed bulk carrier 'John D. Leitch' (IMO: 6714586) at Goderich, Ontario, in position 43 44 43 N, 081 43 36 W, on Feb 16, 2022, at around 02.45 a.m. EST. The tug suffered some damage.

Report: Inadequate lookout contributed to allsion of the "Vachon" in Port-Cartier

Inadequate lookout has contributed to the allsion of the "Vachon" while assisting the bulkcarrier "Orient Crusader" with the breakwater of Port-Cartier, on Sep 12, 2014, concluded an investigation report of the Canadian Transportation Safety Board (TSB). The tug "Vachon" hit the breakwater as it helped the bulk carrier to dock. The report listed a number of causes and factors that contributed to the incident, including inadequate lookout of a part of the crew of the two boats. The TSB noted that the bulk carrier deviated from the recommended path due to the effect of the current. The corrections made were not sufficient to bring the vessel back on the recommended path. No one was hurt in the incident, but the tug and breakwater suffered minor damage.

Collsion with bulkcarrier

The Transportation Safety Board of Canada (TSB) has deployed a team of investigators to Port-Cartier, Québec, after the "Vachon" collided with the bulk carrier "Orient Crusader" (IMO:9464596), during the berthing maneuvers in Quebec’s Port-Cartier in the Saint Lawrence Gulf on Sep 15, 2014. The TSB was gathering information and assessing the occurrence.
