Uncertified master in charge
The "Ken Mackenzie" had an uncertified master in charge when it hit the Queensboro rail bridge. He did not have any marine certification and is continually running boats that require certification - including the "Rustler", "Viking Monarch", "Cindy Mozel" and "Allison Nicole".
Ken Mackenzie struck bridge
A gravel-loaden barge being pushed by the "Ken MacKenzie" allided with a bridge on the Fraser River in New Westminster. The ships struck a railroad bridge that connects Queensbourh to New Westminster near Vancouver east of the Queensborough Bridge on June 28, 2011, around 4:42 a.m. The pair of vessels were heading downstream when it struck the swing bridge causing damage the pivot mechanism. The bridge was stuck in open position and had to be taken out of service. River traffic was halted as the barge was kept wedged against the bridge until the bridge could be stabilized. Early reports indicated that the damage to the train bridge may be extensive, however that will be determined by the authorities conducting a thorough examination of the train bridge structure.
