Inquest in death of tug master
On July 6, 2015, a coronial inquest into the death of Dudley Jacobs in the sinking of the "Adonis" in Gladstone started. The captains of the two tugs involved in the accident have given conflicting evidence on speed or if one told the other to slow down. Capt Jacobs, 64, an experienced skipper and Gladstone marine pilot, drowned in the capsize near the mouth of the Calliope River on June 11, 2011 when the "Adonis" and the Sea Swift tug "Wolli", contracted to Bechtel, had a water barge under tow. In evidence before the Coroner, David O'Connell, the issue of girding was raised where the under-tow barge began overtaking the "Adonis" causing the tow line to be pulled at 90 degrees just prior to the "Adonis" being pulled over and sinking. A safety hook mechanism to allow the tow line cable to be dropped if there was a problem had been in the locked position and not released. The Maritime Safety senior investigator Kevin Schinder said a Marine Execution Plan (MEP) was done that specifically outlined what each vessel was meant to be doing. This was not adhered to and instead a line put over the stern of the "Adonis". The "Adonis" skipper Sean Beardon said the tug had quite a tendency to lean over more than other boats because of its narrow beam. Dead crew member recovered
Gladstone Water Police were searching for a missing man after the "Adonis" capsized in the Gladstone Harbour June 11th, around 12.50 p.m. The police received information that the ship had capsized with four men on board at the mouth of the Calliope River. The "Adonis" was towing a water barge in the Gladstone Harbour when it lost its steering and rolled over and sank. Water Police were able to rescue three men but were unable to locate the fourth man, which is the captain onboard. Police have set up a 100 metre exclusion zone around the tug boat as they continued the search. Police divers reached the scene later tonight. The Water Police meanwhile have confirmed that the body of the man missing in Gladstone Harbour has been recovered.
